Connect
Call Flynt Medical or ask your healthcare provider to send a referral.
You can contact us directly to discuss care for yourself or a loved one, or a healthcare provider may refer a patient who could benefit from in-home primary care.

Verify
We review service-area availability, insurance, and patient eligibility.
Before care begins, Flynt Medical confirms whether the patient is within our coverage area and reviews eligibility, insurance, and clinical need.

Visit
A Flynt Medical provider comes to the patient’s home for primary care.
When appropriate, care is provided in the home, helping reduce the stress and barriers that often come with traveling to a traditional clinic.

Stay Connected
We coordinate follow-up care and, when appropriate, remote monitoring.
After the visit, Flynt Medical helps coordinate ongoing care with the patient, family, and healthcare team. Remote Patient Monitoring may also be used when clinically appropriate.
